How do I get into the Skilled Swing Bed/Transitional Care Program? Once your physician determines that skilled care is needed, a referral will be made to the patient care coordinator or social worker at the Redwood Area Hospital. They will contact your local physician and  determine skilled swing bed/transitional care eligibility and availability.
