How do I qualify for skilled swing bed under the Medicare program or transitional care under private insurance?


Under Medicare, a consecutive, 3-day, acute inpatient hospitalization immediately prior to skilled swing bed care is required.  Under private insurance, admission requirements vary according to individual plans.

The patient must need some form of daily skilled nursing or skilled rehabilitation service which can only be provided in a skilled nursing facility.

Examples of skilled care include:
IV therapy
Sterile dressing changes
Skin/wound care
Rehabilitation therapy: physical therapy, occupational therapy, speech therapy and respiratory therapy.

Efforts will be made to assist the patient to achieve an optimum level of function and self care. The goal is to help the patient return to their home independently or with the help of other community resources such as: home health care, Lifeline, or outpatient therapy.
