RAH Patient Safety Moderate Risk High Risk Fall Prevention at Home

Safe From Falls Initiative Redwood Area Hospital joined the Minnesota Hospital Association SAFE from FALLS Initiative when it began in May 2007 to help avoid patients falling.  The campaign was built on the same successful model as Safe Skin. Safe from Falls is a comprehensive Falls prevention program that addresses the following components to help prevent falls: S - Safety Coordination - Falls Prevention Program A - Accurate and Concurrent Reporting F - Facility Expectations, Staff Education & Accountability E - Education for patients and families F - Fall Risk Screening A - Assessment of Risk Factors L - Linked interventions L - Learn from events S - Safe Environment At the Redwood Area Hospital a falls assessment is completed on all hospitalized patients upon admission.  Patients are assessed based on the following 7 areas: patient age, fall history, elimination, medications, patient care equipment, mobility, and cognition. Falls program interventions are implemented for patients that are considered moderate risk or high risk.  In October of 2008, Redwood Area Hospital, along with 25 other Minnesota hospitals, was recognized by the Minnesota Hospital Association for the implementation of our SAFE from FALLS initiative.
